What they are looking for :
- A Bachelor's degree or higher in Science in Allied Health Professions in Physical Therapy from an accredited program required
- Current license in Physical Therapist in the State of Ohio required
- Preferred two years of community / home health experience as a physical therapist
- Current CPR (AHA BLS) certificate required
- Valid Ohio’s driver license and maintain motor vehicle insurance
Hours and compensation potential :
Position is for full time, 8am-4 : 30pm.Range is between $70K-$102K depending on experience. Full benefit package including a 25K sign on bonus.What you will be doing :
The Physical Therapist is responsible for providing physical therapy services to home health patients. Responsible for supervision of Physical Therapy Assistants. Responsible for being knowledgeable of standard of practice of Physical Therapy in a home health setting.Provides physical therapy services to patients according to a written physician’s plan of care. This may include, but will not be limited to :Assessing and evaluating therapeutic / rehabilitative / functional status, and participation in the development of the total plan of care. Evaluates home environment.Assesses for muscle strength, mobility, gait, ROM – potential for rehab.Directing physical therapy treatment.Instructing patients and families / caregivers in the use and care of therapeutic appliances.Determining priority needs for physical therapy.Reporting to physician patient’s reaction to treatment or changes in condition.Completing medication reconciliation on therapy only patients as needed (on admission to home health and when any changes occur).Completes OASIS charting within three (3) days.Initiates physical therapy program and instructs other personnel and / or family / caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.Complete all charting for each visit within 72 hours.May train patient in the use of prosthetic device.Identifies patient and family / caregiver needs for other home health services and refers as necessary.Prepares and submits clinical and progress summaries based on the attainment of goals.Participates in discharge planning for patient.Additional info :
