Position Responsibilities :
- Assist individuals in developing new skills and achieving their goals as outlined in their Person-Centered Plan (PCP).
- Provide and maintain a safe, supportive environment for clients at home and in the community.
- Use positive reinforcement and behavior modification techniques to encourage growth.
- Help clients with home maintenance and daily living activities as prescribed in their plan.
- Complete accurate documentation of services provided.
- Collaborate with a Team Leader and IDD Program Director for guidance and support.
