The Behavioral Health Consultant, as part of the primary care treatment team, identifies, triages and manages primary care patients with medical and behavioral health problems. This includes providing skill training through psycho-education and patient education strategies and developing specific behavioral / therapeutic treatment plans for patients. The BHC also assists primary health care providers in recognizing and treating mental disorders and psychosocial problems, detecting "at risk" patients, and assessing the clinical status of patients referred by the primary care provider.
Responsibilities :
- Assist primary health care providers in recognizing and treating mental disorders and psychosocial problems (Social Determinants of Health).
- Assess the clinical status of patients referred by primary health care providers and triage to the appropriate resource.
- Provide assessment, consultation, and intervention for patients with chronic emotional and / or health disorders.
- Work with primary care team to treat and manage patients with chronic emotional and / or health problems efficiently and effectively.
- Assist with behavioral health diagnosis and reduce stigma by helping to increase primary care provider comfort with behavioral health conditions and associated treatment.
- Assist in the detection of “at risk” patients and development of plans to prevent further psychological or physical deterioration.
- Evaluate patient care plans with primary care team.
- Educate patients, families, and staff about care, prevention, and integrated treatment enhancing techniques.
- Complete timely and appropriate documentation in the Electronic Health Record (EHR) system, including but not limited to, contact logs, progress notes and evaluation notes.

The pay range for this role is $98,000 - $130,000 on an annual basis.
Locations available are San Marcos, Vista and Oceanside. 3 days in office and 2 days remote schedule.
