The Telephone Triage Nurse assures that communication with each caller will be handled in a professional and tactful manner. When appropriate, the caller’s symptoms will be assessed and triaged using the nursing process and approved guidelines to assist in obtaining the appropriate level of care and/or self-care advice. This position reports to the Director of Nursing.

Duties & Responsibilities:
- 70 % Triage and responds to both symptom and non-symptom related phone calls
- 10% Communication, Customer Service, and technical Skills
- 10% Assures proper documentation and /or escalation of patient safety issues/concerns
- 10% Participates and Collaborates with the Team
- Performs the initial triage assessment of all patient related calls to decide the nature and urgency of the call.
- Collects other subjective and objective information from the medical record based on the assessed need of the call
- Selects and uses the appropriate algorithm(s) from the Triage protocols and guidelines.
- Evaluates and assesses patient’s health care needs based on patient’s signs and symptoms using established triage protocols and guidelines.
- Obtains proper input and direction from the provider as needed to determine the best clinical course of action and recommendations for specific patient health care needs.
- Demonstrates good judgment and clinical thinking skills to respond to patient requests.
- Adapts communication style to person calling who may stand for diverse cultural and professional backgrounds
- Accurately documents symptoms/complaints, nursing assessment, advice provided and patient/caller response.
- Follows policies, procedures, and protocols to ensure consistency and departmental
- Documents in electronic health record (EHR), triage assessment, plan, implementation, and evaluation, follow-up
- Completes other duties as assigned.
- High School diploma or equivalent
- Graduate of an accredited California School of Professional Nursing
- At least one (1) year’s clinical experience in an outpatient care setting
- Availability to work a variable schedule, extended hours and/or Saturday shifts
- Computer proficiency, particularly with Electronic Health/Medical Record systems and the Microsoft Office suite, including Outlook and Word
- Bachelor of Science in Nursing (BSN) degree
- Bilingual in English and Spanish (may be required at some clinics)
- At least one (1) year’s clinical nursing experience in an outpatient care setting
- Previous experience with triage either on the telephone or in an emergency room
- Strong assessment skills
- Excellent clinical skills
- Crisis intervention skills
- Teaching ability, as patients may require instructions for self-care during telephone conversations.
