Position Definition & Summary:
The Transition Services Manager will be responsible to provide the coordination of discharge planning and to ensure the youth's transition plan includes the necessary community resources and family supports required for a successful transition back into the community.
Position Requirements:
· A bachelor’s degree from an accredited university or college in the field of social work, psychology, sociology, criminal justice, or related human services field and at least one (1) year of experience in case management or service coordination.
Job Type: Full-time
Pay: $46,400.00 per year
