About the Role

As a Primary Care Physician, you will be at the heart of participant care, guiding a team dedicated to helping seniors thrive. In this role, you’ll deliver personalized medical care, coordinate treatment plans with an interdisciplinary team (IDT), and manage patient transitions across various care settings. Key Responsibilities

• Conduct comprehensive in-person assessments and create individualized care plans in partnership with the IDT.
• Provide continuous primary care and medical coverage, including discharge planning from hospitals and long-term care settings.
• Support participants in nursing homes through regular visits and assessments.
• Participate in a rotating on-call schedule (nights and weekends).
• Mentor and support Nurse Practitioners and IDT members to maintain exceptional standards of care.

Qualifications

• Doctorate Degree (MD or DO) with completion of an accredited residency program.
• Board Certification in Internal Medicine, Family Medicine, or Geriatrics (preferred).
• Active California Physician License, DEA registration, and State CDS Certification.
• Valid driver’s license and clean DMV record.
• 1-2 years of post-residency experience (preferred).
