Job Description
We are seeking a Program Coordinator with DODD Experience for our Locus & Wood Counties Residential locations and the individuals we serve. We are a company committed to growth and we are looking for a Program Coordinator with the same mindset!
We're looking for a Program Coordinator who is currently a Home Manager, Independent Provider or Direct Support Professional with at least 3 years' experience. Starting out, this role will involve doing 20-30 hours of DSP/IP work and 10-20 hours of administrative and intake work. Upon stabilization of the residence (usually within 30 days), they will work with Human Resources to find a replacement and then seek to add more residential areas. As part of the initial setup process, this position may have to cover call-offs, quits, and PTO requests.
Over the next year, we aim to significantly expand into Locus County. All of our growth goals should be met in your designated area!
Duties
This list includes but is not limited to:
- DSP and Home Manager Functions
- New Client Intakes, Proposals and Meetings
- ISP Development & Training
- Build Case Load for New Clients
- Monthly MAR Updates for Clients
- Hiring and Interviewing of Direct Support Staff (DSP & Home Management)
- Staff Training and Scheduling
- Supervision of Staff
- Quarterly Staff Meetings
- Back-up Coverage for Home Management & Staff
- Employee Certification Tracking
- Review and Approve Timesheets Weekly
- Filing and Reporting of Documentation & Reports
- Reporting MUI/I on by Deadlines
- Preparation of Client’s HPC Documentation/POC
- Completing Internal Compliance Review
- Open Availability to be On-Call and Answer Emergencies and Contact
Requirements
- Proven experience in senior care or related fields such as home health or long-term care.
- Knowledge of care plans, meal preparation, and cooking for seniors is highly desirable.
- 2-3 years experience as a home manager, independent provider for individuals, or home health
- 1 years as a program coordinator, is preferred
- Must keep up on all certifications through the course of employment
- Valid Driver's License & car insurance
- CPR & First Aid Certification (will train)
- Medication Administration 1, 2 & 3 (will train as needed)
- DODD Annual DSP Training (will train, but can complete for free at the Mylearning.DODD.gov website)
- Must be willing to set the standards for excellence in action and training new employees
- A or healthcare is a plus but not required.

Schedule:
- Can largely depend upon the clients, but open availability is often required for this position in the beginning, ultimately it will be a standard Monday-Friday schedule with the necessity to be on-call to handle emergencies and communication from home managers and staff, as needed.
- If called in or had to provide coverage, can flex your schedule during the week to leave early or come in late that week.
