**Shift: 8:30 AM - 5:00 PM Experience:• 12 months of Home Health experience is required.
• 3 months of experience with Epic is preferred.
Requirements:
• Candidates must have a license (required for submission).
• Local & travel allowed. Candidates living <50 miles from facility will be processed as local.
• COVID vaccination required after submission. Religious and medical declinations accepted.

Additional Details:
40 Hour Guarantee
Weekend Requirements: One Weekend Per Month Rotation
On Call Requirements: No Call
Active Drivers License, Auto-Insurance, Reliable Vehicle, and clean DMV Report - Required
At least one year of Home Care experience - Required
Mostly Outpatient w/ Acute Care
Ortho, Neuro, Peds Experience - Required

Dress Code: Uniform or dress/casual clothes no jeans
Coverage Area: 6 counties, typically one county per OT
2 references from last 24 months (Manager/Supervisor reference type) - required for submission

• BLS (Basic Life Support)
