Travel (Social Worker) - Long Term Care

• 12 months of role experience is required from any number of months.

• 10 months of Long Term Care experience is required from any number of months.

• Experience with Epic is required from any number of months. Requirements:

• Candidates must have a license (required after submission).

• This role may require floating to additional units and locations

• Local & travel allowed. Candidates living <50 miles from facility will be processed as local.Pre-employment modules may be required for this role. All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility. References must cover at least 1 full year of relevant experience in specialty within the past 3 years Shift: Mon-Fri; Weekends not required - Daily Patient Load: 1:50-60 (1:1 patient care and group counseling) - Weekend REQ: No - Skills REQ: Biopsychosocial assessment, Documentation and Care Planning - Is on-call REQ? No - Open to accommodating block schedule? Yes; Mon-Fri schedule - Special requirements - Candidate is expected to have reliable transportation and no additional scheduling restrictions. All travel pairs will be required to have their own transportation. - Department Details & Patient Diagnoses: Serving adults 55 and older who are low income, medically complex, have mental dx, behaviors, and/or substance abuse. - Attire / Scrub Color: Any basic color scrubs BLS must be AHA (Instructor, Provider, or RQI) or “Heart&Stroke” certification from Canadian Heart & Stroke Foundation ONLY.
