Job Summary: We are looking for a dedicated and compassionate Home Health Registered Nurse (RN) to join our team. This is a part-time position, requiring 8 hours per week, either in two 4-hour shifts or four 2-hour shifts. In this role, you will provide high-quality, patient-centered nursing care to individuals in their homes. You will work closely with patients, their families, and other healthcare professionals to create personalized care plans that promote health, mobility, and overall wellness.
Key Responsibilities:
- Patient Care and Education – Provide hands-on care to patients and educate them and their families on medical treatments and self-care.
- Health Promotion and Treatment – Implement care plans focused on promoting health and preventing further health issues.
- Monitoring and Documentation – Regularly monitor patients' conditions and maintain accurate documentation of care provided.
- Compliance and Emergency Response – Ensure compliance with healthcare regulations and respond effectively in emergency situations.
- Professional Development and Interdisciplinary Collaboration – Participate in ongoing professional development and collaborate with other healthcare professionals.
Qualifications:
- Education: Bachelor of Science in Nursing (BSN) preferred, or Associate Degree in Nursing (ADN)
- Licensure: Must hold a valid and active Registered Nurse (RN) license in the state of New Mexico.
- Previous nursing experience in home health, hospice, or community health preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills to interact effectively with patients, families, and healthcare team members.
- Ability to work independently and manage time effectively in a home health setting.
