SUMMARY/OBJECTIVE:
The Case Manager coordinates and monitors the assessment, treatment planning process, and follow-up for each child on their assigned caseload. The Case Manager serves as an advocate and liaison for and on behalf of the foster children and foster parent’s rights. The Case Manager ensures foster homes are safe and in compliance with RCCL Minimum Licensing Standards, YFT (Youth for Tomorrow) Standards, and Trusted Family Foundation (TFF) policy and procedures.
ESSENTIAL DUTIES AND RESPONSIBILITIES.
· Maintain confidentiality and other responsibilities listed in governmental laws and regulations, as well as TFF policy and procedures.
· Schedule, coordinate, and visit placed foster children at least once a month or as per minimum standard requirements in the foster home setting.
· Serve as a liaison with the community on behalf of the agency. · Request and receive all records related to the foster client.
· Ensure completion of all required paperwork as per minimum licensing standards.
· Develop and participate in treatment plan meetings with stakeholders involved in the foster client’s case.
· Serve as an advocate for the foster client on behalf of the agency.
· Assist with intake placement decision, schedule pre-placement and placement dates. · Coordinate with staff members to ensure receipt of all placement paperwork.
· Assist in performing all tasks necessary to achieve the organization’s mission. · Effectively and clearly communicate potential risks in a timely manner.
· This position requires long hours, on-call responsibilities, and some travel. · Ensuring that all required foster parent training hours/ courses remain compliant.
· Ensure that all required training is completed as per minimum standards.
· Ensure that valid driver’s license and insurance is always kept on file. REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES Knowledge of: · DFPS Contracting Standards.
· RCCL Licensing Standards. · Organizational development, strong leadership, and program operations.
· General office software, particularly the Microsoft Office Suite, Extended Reach, and use of databases. Ability to:
· Foster and cultivate relationships with foster families.
· Recognize and be responsive to the needs of all foster clients and foster families of the branch office.
· Communicate effectively in both written and verbal form.
EDUCATION AND EXPERIENCE
· Completion of a bachelor's degree at an accredited college or university with two years of experience in Child Placement Agency.
· Excellent verbal, written, communication, and organizational skills. · Excellent people skills with an ability to partner with a dynamic leadership team.
· Possess personal qualities of integrity, credibility, and commitment to a corporate mission.
· Flexible and able to multitask, can work within an ambiguous, fast-moving environment, while also driving toward clarity and solutions; demonstrated resourcefulness in setting priorities, and guiding investment in people and systems
Job Types: Full-time, Part-time
Pay: $19.00 - $22.00 per hour
