**Experience:• 12 months of role experience is required from last 3 years.
• 12 months of Home Health experience is required from last 3 years.
• Experience with Homecare Homebase (HCHB) is required from any number of months.
• Experience with Oasis is required from any number of months.
Requirements:
• Candidates must have a Ohio license or compact license (required after submission).
• Local & travel allowed.Pre-employment modules may be required for this role. Please upload any certifications or health documents you have to your profile to expedite your on-boarding process.
Additional Details:
Monday - Friday. Work every 3rd weekend (day off during the week).
Rotation of on call one weekend to every 3rd weekend. Monthly on call of 2-3 times per month.
Area of coverage includes 10 counties: Allen, Auglaize, Putnam, Hancock, Hardin, Logan, Shelby, Mercer, VanWert, Paulding - the territory does not go to the very edges of those counties. Within a 45 min radius around Mercy Hospital in Allen County.
