Job Overview:
The Community Health Worker for People Who Use Drugs (CHW for PWUD) is a non-clinical, but well-trained individual, a person who is often with lived experience related to Substance Use. The Community Health Worker uses collaboration and a Motivational Interviewing approach with their clients to meet the client’s goals. The Community Health Worker becomes a local resource expert who provides screenings, referrals to treatment, medical care and other community-based organizations to meet their client’s needs. The CHW also often acts as an advocate on the behalf of their clients. Truth Pharm’s CHW for PWUD will work within the values of Harm Reduction, they will have or receive the CCAR Recovery Coach training, Motivational Interviewing, SBIRT, HIPAA Compliance, Boundaries and Ethics and other related training in order to perform well in their roles. CHWs obtain medical consents, collect private health care information, perform assessments, meet with members, make phone calls to community providers, set up appointments and make referrals. A CHW’s role is less intensive than a case manager and is considered a touch point for clients.
Typical and Routine Tasks:
- Client intakes and assessments
- Meeting with clients, determining their needs and goals and assisting them to work towards meeting their goals and needs
- Making calls to or receiving calls from clients
- Making calls on behalf of clients
- Making notes from all client interactions
- Entering client notes in medical records software
- Researching and finding resources to meet client needs
- Building relationships with other community-based organizations
- Finding preferred types of treatment for clients and making direct plans for placement
- Assisting clients in making connections to care
- Educating clients on self-advocacy
- Advocating for clients with other agencies and systems
- Record keeping and data tracking
- Obtaining medical consents
- Submitting Narcan training data to NYS Department of Health
Important Attributes, Skills and Abilities:
- Communication skills – written, verbal and phone
- Active listening skills and willingness – typically clients should be speaking more than the community health worker
- Computer skills – record keeping software, Microsoft Office and Google documents (including light spreadsheet work)
- Strength to advocate for others remaining professional while being firm
- Ability to work in a team environment to problem solve
- Good judgment when making decisions regarding clients
- Ability to work with people from various backgrounds
- Sensitivity to diversity and cultural differences
