Work Schedule: Monday through Friday, Morning, Evening & Night shifts – Full time / Part time available.
Pay Rate: $35-$40/hr (Strictly between the range)
Job Description:
The Licensed Practical Nurse (LPN) will provide basic nursing care under the supervision of registered nurses and physicians.
The LPN will be responsible for monitoring patient health, administering treatments and medications, and ensuring patient comfort and well-being. This role requires close collaboration with a multidisciplinary healthcare team to provide high-quality care in line with established nursing practices and procedures.
Qualifications:
- Education: Completion of an accredited LPN program.
- Licensure: Active and valid LPN license in the state of New York.
- Certifications: Current BLS/CPR and First Aid certifications required.
Key Responsibilities:
- Provide direct patient care, including administering medications, performing dressing changes, and monitoring vital signs.
- Assist patients with daily living activities such as bathing, dressing, and mobility.
- Observe and monitor patient conditions, reporting significant changes to the Registered Nurse (RN) or physician.
- Administer injections, IV treatments, and other procedures as prescribed.
- Accurately record patient information, including health status, treatment plans, and responses to interventions, in medical records.
- Ensure timely and accurate completion of all documentation in compliance with healthcare regulations and facility policies.
- Educate patients and families on health conditions, treatment plans, preventive care, and self-care.
- Collaborate with RNs, physicians, and other healthcare professionals to develop and implement individualized care plans.
- Ensure effective communication with the healthcare team to support continuity of care and improve patient outcomes.
- Adhere to infection control protocols and maintain a clean, safe environment for patients.
- Follow state and federal regulations, as well as facility policies, in all aspects of patient care and documentation.
- Participate in continuing education programs to stay current on nursing practices.
- Advocate for patients, ensuring their dignity, privacy, and rights are respected.
- Provide compassionate emotional support to patients and families.
Required Skills and Abilities:
- Proficiency in basic nursing techniques and procedures.
- Strong ability to monitor and document patient vital signs and care activities.
- Competence in administering medications and performing treatments as prescribed.
- Excellent verbal and written communication skills.
