Job Details
Incumbent serves as an Outpatient / Inpatient Social Worker / Registered Nurse (RN) Case Manager for TCRHCC and Sacred Peaks Healthcare Center (SPHC) and is responsible for direct supervision and supportive contact for the specific group of identified high-risk patients.
Responsible for designing and managing a continuum of care focusing on empowering clients to achieve demonstrable outcomes and self-sufficiency.
Responsible for assessment, service planning, and resource acquisition, monitoring progress and initiating and responding to emerging client needs.
Responsible for utilization review of patient cases in the inpatient and outpatient services departments.
Job Requirements
Associates degree in Nursing, must obtain bachelors in nursing within 2 years of hire.
Must have and maintain current Basic Life Support (BLS) certification by the American Heart Association throughout employment.
A valid, current, full and unrestricted Professional Nursing License to practice nursing in any state of the United States of America.
Schedule Information
Orientation : Every two (2) weeks only.
Unit Specific Information
The nursing staff includes registered nurses, licensed practical nurses, nursing assistants, community health nurses, nurse practitioners, nurse midwives, CRNA, and community health medics.
Additional Details
COVID EXEMPTIONS : We do not accept religious exemptions. If a candidate submits for a medical exemption, we will have to get approval from our medical review board to proceed with onboarding.
