JOB SUMMARY/OBJECTIVE:
The Licensed Practical Nurse (LPN) or Registered Nurse (RN) works under the supervision of the Director of Nursing/Program Lead and is responsible for the complete care of all assigned patients, within the scope of the State Registered Nurse Practice Acts and the policies and procedures of the Agency. The LPN is responsible and accountable for making decisions and providing care according to the Oregon Board of Nursing regulations and agency guidelines.
This position will be responsible for providing medical screening for medical appropriateness for services within the center, services to individuals, ensuring the overall medical health of individuals are consistent with protocols outlined in the policy and procedures, and making overall medical decision of the welfare of individuals being referred the Multnomah County Deflection/Sobriety Center.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Follow-up with any patients in need of medical follow-up as determined by review of medical file/documentation.
- Keep abreast of all State standards for nursing practice in an intermediate care facility and to keep in compliance with these standards at all times.
- Perform the Nursing Assessment for each individual and determine the individual’s appropriateness for services within the deflection center.
- Document assessments, interventions, and acute changes in client behavior and condition.
- Direct and supervise staff considering scope of practice.
- Maintains all medical forms in clinical records in complete and timely manner.
- Review and complete patient paperwork as needed.
- Monitor clients' complaints and incidents; reply in accordance with agency/regulatory body guidelines for complaint tracking procedures and policy.
- Understands the center is open twenty-four hours and ensures accessibility to facilitate safe and adequate care; on-call responsibility may be required for this role.
- Responsible for follow-up of all incidents and incident reporting. Works with staff, as necessary to resolve incidents.
- Responsible for monitoring and follow-up on Infection Control/Surveillance reporting.
- Provide for a comprehensive and coordinated program of client care services in con junction with agency policies.
- Maintain a system of client care records; reporting forms, client and service data and any other needed client care information as required.
- Ensures that all client care services are coordinated and integrated as indicated.
- Assists with the development of clinical education programs.
- Maintains all needed clinical documentation from doctors, nurses, etc.
- Keeps abreast of changes in State and Federal Regulations, Joint Commission standards and contractual requirements related to any aspect of Clinical Compliance.
- Ensures quality and safe delivery of services.
- Maintains a multi-disciplinary approach to delivery of services.
- Attends meetings as necessary or required.
- Complete intake duties and responsibilities as assigned based on staffing needs.
- Serve as a charge nurse when assigned, delegating tasks and supervising staff as appropriate to their scope of practice.

REQUIRED EDUCATION, CERTIFICATIONS AND/OR EXPERIENCE:
- High school Diploma or GED equivalent required.
- Valid Oregon RN or LPN license required.
- BLS Certification (Required or the ability to obtain within the first 90 days of employment).
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
- Demonstrated ability to use entire MS Office (Outlook, Word, and Excel). Ability to learn in-house software.
- Ability to work and follow-up on Joint Commission and/or DHMH survey preparation and subsequent recommendations.
- Demonstrated ability to maintain the client's right to courtesy and confidentiality.
- Consistently and promptly performs all monitoring as prescribed by Infection Control Policy.
- Demonstrates thorough familiarity with all Rules and Regulations governing client behavior.
- Personal belief in recovery and the recovery process.
- Excellent written and verbal communication skills, with the ability to interact effectively with staff, clients, and the public.
- High energy, flexibility, and strong interpersonal skills to work effectively within a team.
- Ability to engage clients flexibly based on their individual needs.
- Respect for and maintenance of professional boundaries with all individuals.
- Ability to follow written and verbal instructions accurately.
- Proficiency in reading, understanding, and speaking English.

Physical Requirements:
- Prolonged periods of sitting at a desk and working on a computer.
- Must be able to lift up to 15 pounds at times.
- Prolonged periods of standing and bending.
