Responsibilities:
- Provide therapeutic care, education, and referral to patients and families.
- Assess patient needs relative to home health aides and communicate with team RNs and HHA Manager.
- Develop, maintain, and apply knowledge of third-party payer guidelines governing eligibility for reimbursable home health services.
