Responsibilities
- Provide clinical social work services to patients and families, focusing on psychosocial assessments and interventions in various care settings.
- Collaborate with healthcare teams, addressing crisis situations, facilitating discharge planning, and ensuring continuity of care while promoting patient dignity and respect.
- Develop and implement individualized plans of care to address the unique needs of patients and families.
- Consult with agency care providers regarding social service needs of clients and families.
Requirements
- Master's degree in Social Work from an accredited institution.
- Licensed Clinical Social Worker (LCSW).
- One year of related experience.
