What are the responsibilities and job description for the Travel Physical Therapist - $2,382 per week position at Tug Valley ARH Regional Medical Center?
Certification Details BLS KY License Job Details Must have 2 plus years of Inpatient PT experience in an acute care setting. Will cover IP Acute care / SNF and LTACH. Orientation is bi-weekly. Start date on job is start date needed. Resource must reside 100 miles from facility to be considered. Assignment confirmation needs to be loaded within 24 hrs of accepting offer. Driver's license is required in to be included in submission packet. Job Requirements KY License BLS required for all RNs, Respiratory Therapists, and CT Tech. ACLS optional for all RNs except Psych and True Med / Surg units. Proof of 2 MMR vaccinations or a positive titer are required prior to start. Positive titer or proof of 3 Hep B vaccine series or declination can be accepted. Proof of 2 Varicella vaccinations or a positive titer are required prior to start. Valid Td vaccine required. If positive TB record; Requires Proof of Chest X-Ray with in last 5 years to be accompanied by TB questionnaire. Schedule Information Sept. 1st-May 31st. Subject to change at facility’s discretion. Religious and Medical exemptions are accepted with proper documentation and approval. Unit Specific Information Coversheet and requirements must be listed along with facility and Unit information : Minimum of 2 years dependent on specialty. Additional Details 2 references required; 1 Charge or Higher; 1 must be supervisor reference within the last year. Annual update required for various credentials.
