Summary
Case Manager will coordinate referrals to resources within the community to further address medical, mental health, behavioral health, and social needs of the patient. When identifying at-risk patients, this position must be able to make critical decisions regarding the patients' mental/physical health needs to include enhanced social work assistance. The position will also assist with the overall mental health component of the Center for Sport. Responsibilities may include: clinical and coordination of services management, patient assessment, treatment planning and coordination, multidisciplinary staffing, case management and ongoing program monitoring and development.
Required Knowledge, Skills, And Abilities

Required Knowledge, Skills, And Abilities
- Clinical knowledge of human development and psychological theory as it relates to traumatic brain injuries, human behavior, addiction, and PTSD.
- Understanding of the traumatic brain injuries and its impact on physical and psychological states and relationships.
- Possess relationship building skills across a diverse range of cultural settings.
- Knowledge and experience in the use of medical and mental health diagnoses, disabilities, and treatment procedures. This includes: acute, chronic, and traumatic illness/injuries; common medications and their effects/side effects; medical terminology. Knowledge and use of the Diagnostic and Statistical Manual of the APA (DSM-5) is required.
- Knowledge of community resources, how to make appropriate referrals to community and other agencies for services, and ability to coordinate services.
- Master's degree in Social Work, Counseling, or Rehabilitation Counseling from an accredited program
- Licensed Master Social Worker or Licensed Professional Counselor or Certified Rehab Counselor in Louisiana (or the ability to obtain before start date)
- Three years experience with mild to moderate TBI, PTSD in direct patient care, case management, and psychosocial issues
- Licensed Clinical Social Worker or Rehabilitation Counselor with 2-3 years direct experience with mild to moderate traumatic brain injuries.
- Experience working with active duty military and/or veterans.
- Experience in mental health rehabilitation and complex medical and neurological conditions.
