TulaRay is committed to providing exceptional patient care services. As a Social Worker 2 (SW 2), you will play a critical role in supporting patients between the ages of 18 and older within an inpatient psychiatric hospital.
Position Purpose:
The Social Worker provides support to patients through direct clinical contact, family/community agency contact, social work consultation to other professional disciplines, and collaboration with the patient's assigned treatment team. The role involves completing admission assessments as assigned and specific duties as assigned by the Social Work Supervisor and Manager to ensure efficient and effective operations of the social work department.
Responsibilities:
- Conduct interviews with individuals, family members, legal guardians, and external agency representatives
- Assess individuals' psychosocial strengths, weaknesses, and social work service needs, and eligibility for services
- Develop and implement comprehensive individual care and treatment plans
- Ensure needed benefits and services are obtained in accordance with assigned individuals' treatment plans and applicable mandates, regulations, and policies
- Apply for Medical Assistance, Social Security, and other benefits for patients
- Provide full-range social work services for an assigned caseload in a mental health facility of 20 patients
Requirements:
- Possession of a degree in Social Work
- Past experience working in the mental health field preferred
