Responsibilities Include:
- Interviewing individuals, family members, and external agency representatives to obtain information regarding physical, mental, emotional, economic, and social status.
- Assessing individual psychosocial strengths and weaknesses, as well as social work service needs.
- Completing annual and initial psychosocial assessments.
- Discussing assigned individuals' psychosocial assessments and family history in multidisciplinary treatment team meetings.
- Participating in the development and implementation of each assigned individual's comprehensive individual care and treatment plan.
