What are the responsibilities and job description for the Telehealth Patient Facilitator (750681) position at TulaRay?
Minimum Qualifications
- Knowledge of Tele-Health processes and procedures is preferred.
- Background in mental health is preferred.
- Previous administration and/or medical assistant training or duties preferred.
- Electronic Health Records background preferred.
- All patient labs are to be scanned and entered into the patient’s chart on a daily basis.
- Retrieve any necessary forms and paperwork needed by the Tele-Health Provider.
- Prepare treatment team rooms for appointment.
- In the absence of electronic health records (system down, etc.), fax, scan, e-mail most recent Progress Note and any other relevant documentation to the Provider prior to the session as well as receive notes and relevant orders from the Provider at the conclusion of the session.
- Ensure completion of all required forms.
- When applicable, coordinate with scheduler for scheduling appointments.
- Assist in orientation and training of new Tele-Health Providers, including Locums, regarding facility operations and procedures.
- Educate the patient on the nature and process of Tele-Health, specifically discussing confidentiality.
- Obtain patient consent when required.
- Ensure all equipment is functional and fully operational.
- Assist with flow of patients in and out of Tele-Health clinic and remain present during clinic sessions.
- Share information as requested by Provider.
- Assist the Provider in using Tele-Health equipment.
- Fax/email relevant documentation to the Provider as well as receive notes and relevant orders from the Provider.
- Ensure the call has been disconnected and the power is turned off to the video conferencing equipment at the conclusion of the Tele-Health session.
- Ensure that notes and pharmacy orders are provided to the Nurse for processing at the conclusion of the appointment. File such notes and orders where appropriate.
- May be required to train other staff regarding the role of the patient facilitator.
