Job Title: Bilingual Intensive Care Coordinator

Job Description
The Bilingual Intensive Care Coordinator plays a crucial role in facilitating and coordinating comprehensive care services for individuals living with HIV/AIDS. This position assesses clients for biopsychosocial care needs; refers, advocates, coordinates, and monitors clients’ needs to improve a client’s retention in care, other health and social outcomes, and client quality of life.
Responsibilities:
Client Assessment:
- Conducts comprehensive initial and ongoing assessments of clients’ medical, social, and emotional needs related to HIV care.
- Identifies barriers to care and develops strategies to address them.
- Upon intake, and once every six months minimum, fully orients each client to the care coordination program objectives, other Tulsa Cares programs and services, and their rights and responsibilities as a client.
Care Planning:
- Generates an individualized care plan based on service need level findings, client priorities, and quality improvement goals through client-centered and motivational interviewing techniques.
- Coaches' clients on engagement in healthcare including chronic disease self-management and adherence, mental health and substance abuse, life skills development (literacy, GED completion or vocational rehab, budgeting and financial planning), healthy relationships, and building sense of life purpose.
Medication Adherence:
- Attempts to establish or to retain each client in regular medical care, utilizing appropriate client-centered and motivational interviewing techniques, regular follow-ups, and interventions.
- Addresses barriers to medication access and compliance which includes HIV Drug Assistance Program application assistance and maintenance, compassionate care applications, and Tulsa CARES prescription assistance program.
- Facilitates enrollment in medical programming and qualified health and prescription plans for eligible clients.
Other Functions:
- Facilitates referrals to specialists, support services, and community resources as necessary.
- Provides disease specific education and secondary prevention/harm reduction education.
- Offers emotional support to clients and their support systems.
- Maintains accurate and up-to-date client records, ensuring confidentiality and compliance with regulations.
- Complies with the Care Coordination program and agency policies and procedures as established by Tulsa CARES.
- Supports an environment of health and wellness in accordance with the agency’s philosophy of care.
- Adheres to the Oklahoma State Department of Health Standards of HIV Case Management Care.
- Works as part of an interdisciplinary team to meet the needs of the entire person.
- Actively participates in formal supervision and bimonthly case staffing and monthly case management meetings.
- Performs other duties as assigned.

Educational and Licensure Requirements
- Degree from an accredited university in social work or a related field (preferred); relevant experience as alternative.
- Must maintain current driver’s license and any relevant licenses or certifications.
Experiential Requirements
- Fluency in Spanish required
- At least 2 years of case management experience preferred.
- Experience working with those living in poverty, with HIV/AIDS, with other chronic health conditions, and/or with history of mental health or substance abuse is preferred.
Knowledge, Skills, & Abilities
- Motivational interviewing and trauma-informed techniques
- Knowledge of database software; Proficient with Microsoft Office Suite (Word, Excel,
Outlook).
- Must have ability to collaboratively work with a variety of health and/or social service organizations.
- Effective written and verbal communication skills; ability to maintain professional records through documentation.
- Time management and organizational skills to prioritize and address multiple clients’ needs.
- Ability to navigate and adapt to diverse community settings.
