As the Social Services Assistant at Tulsa Nursing Center in Tulsa, OK , you will be a member of the facilitys interdisciplinary team. The Social Services Assistant assists the Director of Social Services in care planning and coordinating referrals to other healthcare providers and facilitates the development of resident discharge plans.

Responsibilities include :
- Assists with coordinating admissions to the facility. This includes re-admissions, room moves, and patient inquires.
- Assists with coordinating discharges.
- May be required to assist with coordinating resident and family member grievances and acts as the resident advocate, when necessary.
