What are the responsibilities and job description for the Social Worker position at Tupper Lake Center for Nursing and Rehabilitation?
Position Summary
Responsible for social work program development, linking and coordinating all aspects of care for the residents' quality of life. participates in policy development and planning for the facility in accordance with State and Federal regulations. Assists residents in meeting and solving their psychosocial and economic needs and problems and makes referrals to appropriate agencies. Serves as an advocate to assure resident rights and assists families in supporting the resident. Works collaboratively with physicians, nurses, and other members of the interdisciplinary team, and government and community agencies to effect positive patient / resident and family outcomes. Expected to address and assist residents with obtaining financial resources. May be required to travel to sites beyond Mercy Living Center for patient services.
Educational Requirements / Qualifications
- Minimum of one year of experience in a health care setting working directly with individuals.
- Long-term care experience preferred.
- Understanding of the discharge planning processin long term care.
- Knowledge of community resources preferred.
