Job Description: Community Coordinator

Minimum Qualifications:
- Bachelor's degree in Psychology/Social Work, valid unrestricted LCSW license, or a Mental Health Counselor
- Current CPR (Healthcare Provider)
- Ability to pass a background check
Responsible for discharge planning for inmates receiving mental health services. Develop and maintain a procedure for compliance with contract requirements for release prescriptions and recurring prescriptions following release.
Review all new admissions and meet with patients and complete discharge planning evaluation to determine all discharge planning needs as indicated by treatment team.
Assist inmates who need follow up in order to continue essential medications, chronic care, and/or their treatment plan by seeking appointments with willing community resources/health providers. Ensures common understanding of admission criteria, eligibility and enrollment requirements when applicable.
Supports inmate clients by providing required applications and assistance in confirming eligibility, as well as by referring them to assigned agency and correctional facility program.
Acts as educational liaison and provides consultation to medical personnel regarding discharge planning process, applicable regulations, and accreditation standards.
Communicate all discharge planning issues with all members of the treatment team.
Communicate all discharge planning with the client, inpatient and outpatient community team.
Responsible for creating and maintaining relationships with community agencies providing services to persons with mental illness.
Develop and utilize a discharge resource book identifying community resources i.e. social service resources available in the community for patients.
Demonstrate accurate and timely documentation according to guidelines.
Prepares and submits statistical data, monthly activity and annual reports as directed.
Assist patients with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner.
Assist in the orientation/training of new Team Members.
Ensure compliance with regulatory standards.
Perform other duties within scope of practice as assigned.
