Help adolescents recover from mental health and substance use challenges at an 18-bed residential treatment center in Woodbury, Connecticut. The Woodbury team includes: psychiatrists, therapists, nurses, wellness coordinators, academic advisors, operations staff, kitchen staff, and more.
Job Responsibilities:
- Perform evaluation and management visits and coordinate care for clients under the direction of supervising psychiatrist.
- Assist with care planning and provide follow up during transitions of care.
- Maintain records of all visits and care coordination activities within the electronic health record.
- Provide support for improving overall wellness, managing clients, and identifying gaps in care within the agency.
- Collaborate with clinical and behavioral staff to provide best care possible.
- Client population will be 14- to 17-year-olds in a residential environment.
- Other duties as assigned.
Job Requirements:
- Active Connecticut license as an Advanced Practice Registered Nurse.
- Minimum of two years of clinical experience as a Nurse Practitioner.
- Residential experience preferred.
- Experience working with co-occurring disorders preferred.
