About the Position :
We prefer that our case managers have experience in a 12-step recovery program (AA, NA, Al-Non, Nar-Anon, Families Anonymous, GA, etc.). We also would like candidates to have worked where clients live, like in a residential program.
We are hiring a case manager for our young women's extended care program, which is a three-phase long-term treatment program for mental health and substance use challenges. The young women's extended care program is composed of highly structured, supervised, and supportive recovery residences where our clients ages 18 to ~27 engage in much programming and personal development, as well as receive clinical services (group, individual, and family therapy, as well as medication management where appropriate) at our downtown New Haven clinic. As above, the program is long-term in nature phase one is typically two to three months; phase two is typically two to three months; and phase three is typically six months or longer. A client has a primary clinician at our clinic, a case manager at our residence, and many other team members to support them.
A case manager works in the residences (either the women's phase one and two house, which is a single location, or one of our three women's phase three houses) and works in collaboration with the clinical team downtown. At the residences, a case manager is supervised by a phase director and works alongside a team of other case managers, wellness coordinators, resident liaisons, and support staff. A case manager also engages fully with clients building rapport with them and their families, getting to know their needs, and helping them have a positive treatment experience. A case manager engages in weekly meetings with clients on their caseload and also completes weekly family progress calls to each client's family on their caseload. While there are many reasons a family chooses Turnbridge for their loved one, a major reason is because their loved one needs an excellent long-term treatment program. The ideal case manager identifies fully with that mission, understanding that long-term treatment helps a young woman build a life worth protecting.
