What are the responsibilities and job description for the Bilingual Community Health Worker position at Tuscarawas County Health Department?
Community Health Workers (CHW) will primarily be working out in the community with specific target populations. CHWs will work closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes. The CHWs will provide links between clients and resources. CHWs will provide education, perform assessments, and coordinate services through home and office visits with clients.
Minimum Qualifications
- High School Diploma or GED required.
- Fluent in Spanish
- 3 years of health/social service experience and/or education.
- Must have completed a formal CHW training or be willing to complete.
- Must have a valid State of Ohio driver’s license, automobile availability, and liability insurance coverage that meets the Personal Vehicle Use Policy.
- Must be nicotine free
