In this role, you help individuals maintain a safe, supportive, and therapeutic environment which fosters growth and rehabilitation. The Community Participation Support and Habilitation Professional will build trust through reliability and communication. A solid ability to manage complex behavioral needs with care, respect, and knowledge. Overall, providing quality care specific for individual needs and aligned with the Department of Human Services "Everyday Lives." This role is crucial in ensuring the effective delivery of services while maintaining compliance with regulatory standards. The successful candidate will be responsible for various tasks that include, but are not limited to:

  • Implement the mission statement and vision statement of the organization
  • Adhere to all company policies, state regulations, and quality assurance
  • Maintain accurate records of service delivery, including Electronic Visit Verification (EVV) logs and location tracking.
  • Participate in training sessions, meetings and workshops to stay up to date with industry standards, but also best practices in EVV and location tracking.
  • Have and use a cellular device with working service in which to communicate and utilize for Electronic Visit Verification (EVV).

  • Identify and assist in risk, quality and incident management
  • Completes all necessary documentation prior to end of shift including timely reporting of all staff, residents, site including any and all incidents.
  • Prepare reports as needed for management to ensure transparency and accountability in service provision.
  • Communicate regularly with Program Management to ensure continuity of care based on ISP and development of ISP
  • Encourages healthy choices.

  • Implement behavior treatment plans and related service outcomes as trained and required
  • Maintain and reconcile client funds accurately and with integrity.
  • Retain and submit all receipts and client documentation to the Program Specialist at the conclusion of each shift.
  • Assist/manage individuals specific needs as outlined in service delivery specifications as outlined in the Individual Support Plan (ISP) which includes but not limited to cleaning, grocery shopping, meal preparation and transporting to appointments/activities.
  • Responsible for the direct supervision of all individuals with developmental and intellectual disabilities and autism within our care.

  • Report any individual abuse, incident, health, safety or act of omission as per Pennsylvania Office of Developmental Program offices Incident Management.
  • Have and maintain a vehicle with current inspection, registration, and proof insurance coverage and acceptable safe driving record.
  • Read and interpret documents with or with reasonable accommodation such as safety rules, manuals, policies, procedures and other programming requirements.
  • Ability to speak effectively before groups of business partners, service providers, parents or other groups of individuals as requirement for the position.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Adhere to all Health Insurance Portability and Accountability Act of 1996 (HIPAA's) rules, laws and requirements including confidentiality, conflict of interests' policies, noncompete agreements to protect the identifies and information of the individuals we support.
  • Must be able to operate equipment (e.g., wheelchairs, hand trucks). Must be able to exert up to seventy-five (75) pounds of force occasionally and/or frequently to lift, carry, push, pull or otherwise move objects.

the overall success of those we support and serve as "natural role models". Communication Participation Support and Habilitation Professionals must have the following personal qualifications:


  • Leads with integrity and exhibits ethical moral
  • Leads with empathy and can de-escalate and redirect behaviors appropriately.
  • Enjoy building collaborative relationships and rapport through trust and clear
  • Have knowledge of basic word processing skills and experience with Microsoft Office products and email usage
  • Be able to utilize the Electronic Visit Verification (EVV) system in Therap to accurately document service delivery times and locations.
  • Ensure all visit logs are completed in real-time and comply with state and federal
  • Participate in training and feedback to ensure quality service delivery as per Individual Support Plan and programming standards.
  • Train and assist supported individuals and caregivers in using Electronic Visit Verification (EVV) system correctly and effectively.

Qualications:


Education and/or Experience

  • A High school diploma or General Education Degree (GED)
  • 1 year of Human Service work experience (preferable)

Other Qualications

  • Minimum age 18
  • Physical exam and TB Mantoux test
  • Not be disqualified on any of the local, state, or federal exclusion lists
  • Have an acceptable child and criminal history, no disqualifying violations
  • Autonomy - ability to work without immediate supervision
  • Must possess a valid PA Driver's License w/ an acceptable driving history
  • Digital management skills (phone, text, tablet)
  • Autonomy - ability to work without immediate supervision
  • Knowledge of the Therap EVV and ISP Data documentation platform
  • CPR/First Aid Certification
