Qualifications:
Bachelor's degree in a human services field required. Experience providing services for children required. Experience in case management preferred. Experience working with children in the foster care system or with behavioral health challenges preferred. We believe our staff are managing the CARE of a child, not a legal case. Job Environment:
Works daily in an office-based setting with a team of other professionals with a focus on the best interest of the child served. Ensuring foster parents are provided with consistent attention and support necessary to meet children's needs. Regular travel within the service area Columbus/LaGrange, visiting with children and foster parents at home and in the community. Agency vehicle is shared between staff, mileage reimbursement allowed if car is not available. Agency cell phone provided.

Responsibilities:
- Provides case management for children with a range of behavioral health, developmental and medical diagnoses who are in foster care program.
- Responsible for ensuring children's medical, dental and mental health needs are consistently met.
- Creates a master service plan to guide foster parents, and assists foster parents with plan implementation.
- When needed, provides support for birth families of children assigned.
- Makes referrals for services to community mental health, dental and medical providers.
- Identifies and works with community resources for needed services and supports, including extra-curricular activities, after school care, summer enrichment programs for children assigned.
- Rotates after hours, holiday and weekend on-call support duties with other case management staff to provide crisis intervention and support related to emergencies for children served.
- Maintains the child record by documenting progress and contacts with children and foster families, filing documents, requesting needed documents and ensuring the file is consistently up to present date.
- Assists foster parents with utilizing each child's clothing allowance by tracking spending and submission of receipts.
- Secures respite placements within the program to support regular breaks for foster parents.
- As primary support for the foster parent, works to solve problems and prevent problems through effective communication and monitoring of placement services.
- Provides a variety of support services for children as needed, such as transportation, child care and advocacy with outside agencies.
- Responsible to consistently communicate and share information with the Department of Family and Children's Services case manager.
