Position Summary
Twin Cities Physicians is seeking a Nurse Practitioner (AGNP / CNP) to join our growing team! In this role, you will bring primary care directly to patients where they call home. Responsibilities
- Drive to senior living communities to provide on-site primary care to residents.
- Facilitate chronic care management, advanced treatment, and end-of-life planning.
- Coordinate care with an interdisciplinary team, reconcile medications, and order labs.
- Communicate regularly with physicians, office staff, community teams, and patient families.
Qualifications
- Certified Nurse Practitioner (CNP / AGNP) licensed in Minnesota
- Experience in geriatrics and/or primary care preferred
- Valid driver’s license and current auto insurance
- Passion for serving the senior population
- Strong interpersonal and communication skills
