What are the responsibilities and job description for the Case Manager position at Twin Rivers Developmental Supports, Inc.?
Twin Rivers Developmental Supports, Inc., is currently accepting applications for a Case Manager. The Case Manager is responsible for counseling and coordinating services for individuals on an assigned case load. This is a full-time position working 8:00 am.to 4:30 p.m. Monday through Friday.
Job Duties:
- Obtain and maintain a license with Health Care Policy to provide Case Management.
- Write and implement plans and programs.
- Serves as contact for parents/guardians and other professionals.
- Abide by the Health Care Policy Rules of Conduct for Case Managers.
- Assist with securing needed support and ancillary services.
- May be required to assist as back up, fill in and behavior management.
- Transporting individuals to and from appointments as needed.
- General office duties.
- Other duties as assigned.
Qualifications:
- Bachelor’s degree (full-time experience in the field of developmental disabilities may be substituted for a degree at the rate of six months full time experience for each missing semester).
- One year of experience in the field of developmental disabilities.
