Position Overview

As a Home Health RN, you will deliver skilled nursing care to patients of all ages in their residences. You’ll assess, plan, and implement care plans while collaborating with an interdisciplinary team to ensure exceptional patient outcomes.

Key Responsibilities

• Conduct comprehensive patient assessments to develop individualized care plans.
• Administer treatments, medications, and wound care according to established protocols.
• Educate patients and families on disease management, self-care, and wellness strategies.
• Document clinical notes and patient progress accurately and timely.
• Coordinate with interdisciplinary teams, physicians, and caregivers to optimize patient care.
• Ensure compliance with home health regulations and standards.
• Provide on-call support for patients as needed.

Qualifications

• Education: Graduate of an accredited nursing program (Diploma, Associate, or Baccalaureate).
• Licensure: Current State RN license and valid driver’s license.
• Experience: Minimum of 1 year in a clinical care setting, home health experience preferred.
• Skills:
  • Strong clinical and problem-solving skills.
  • Excellent communication and interpersonal abilities.
  • Proficiency in home health documentation and electronic medical records.
  • Current CPR certification.
• Transportation: Reliable vehicle with valid auto liability insurance.

Work Environment

• Deliver care in patients’ homes with a flexible schedule.
• Travel locally to serve patients in your community.
• Physical activity includes prolonged standing, walking, and lifting up to 50 lbs.
