Position Overview
As an RN Case Manager, you will provide skilled nursing care to patients of all ages in their homes. You will coordinate with an interdisciplinary team to ensure patients’ physical, psychosocial, and emotional needs are met with excellence.
Key Responsibilities
- Conduct initial and ongoing comprehensive assessments of patients’ needs, including physical, functional, and psychosocial evaluations.
- Develop, implement, and regularly update individualized plans of care (POC).
- Educate patients and families on disease progression, self-care techniques, end-of-life care, and nutritional needs.
- Coordinate and prioritize services as part of an interdisciplinary team approach.
- Maintain detailed clinical and progress notes to document patient outcomes.
- Perform on-call responsibilities and provide support to patients and families as needed.
- Ensure compliance with hospice care criteria and maintain appropriate levels of care.
Qualifications
- Education: Graduate of an accredited Diploma, Associate, or Baccalaureate Nursing program.
- Licensure: Current State RN license and valid driver’s license.
- Experience: At least 1 year in a clinical care setting, hospice experience preferred.
- Skills:
- Strong interpersonal and clinical nursing abilities.
- Specialized skills in palliation and end-of-life care preferred.
- Proficiency in English (verbal and written).
- Current CPR certification.
- Transportation: Reliable vehicle with valid auto liability insurance.
Work Environment
- Provide care in patients’ homes under varying conditions.
- Potential exposure to blood, bodily fluids, and infectious diseases.
- Flexible scheduling with local travel and PRN emergency call responsibilities.
- Physical activity includes prolonged standing, walking, and lifting up to 50 lbs.
