U Got A Friend (UGAF) Premium Veteran and Senior In-Home Care was founded in 2019 and is headquartered in Tulsa, OK. We are seeking a highly motivated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for coordinating and advocating for the needs of individuals and families who require assistance in navigating various social services and healthcare systems. You will assess client needs, develop care plans, and collaborate with a multidisciplinary team to ensure the provision of comprehensive and quality services. The ideal candidate will possess excellent communication and organizational skills, along with a strong commitment to improving the lives of others.
Duties and Responsibilities:
- Conduct initial assessments of clients to determine their needs, goals, and eligibility for services.
- Develop individualized care plans in collaboration with clients, their families, and other professionals involved in their care.
- Coordinate and monitor the implementation of care plans, ensuring the provision of appropriate services and resources.
- Advocate for clients' rights and access to necessary services, acting as a liaison between clients and service providers.
- Provide information and support to clients and their families regarding available community resources, benefits, and entitlements.
- Regularly evaluate clients' progress and modify care plans as needed, in consultation with the interdisciplinary team.
- Collaborate with healthcare providers, social workers, counselors, and other professionals to ensure coordinated and holistic care for clients.
- Maintain accurate and up-to-date documentation of client interactions, progress notes, and service plans in accordance with organizational and regulatory requirements.
- Participate in case conferences, team meetings, and trainings to enhance professional knowledge and skills.
- Adhere to ethical standards and maintain client confidentiality at all times.
Required Skills and Qualifications
- Must be a Certified ADvantage Case Manager. (Please do not apply you are not a Certified ADvantage Case Manager)
- At least 2 years of proven experience as a Case Manager.
- Bachelor's degree in a related field (Master's degree preferred).
- Knowledge of social services, healthcare systems, and community resources.
- Strong assessment and care planning skills, with the ability to develop individualized plans based on client needs.
- Excellent communication skills, both verbal and written, with the ability to effectively collaborate with diverse individuals and teams.
- Empathy, compassion, and the ability to build rapport with clients from diverse backgrounds.
- Strong organizational and time management skills, with the ability to prioritize tasks and meet deadlines.
- Familiarity with relevant laws, regulations, and ethical standards in the field of case management.
- Proficiency in using computer systems and software for documentation and data management.
- Valid driver's license and reliable transportation, as extensive travel (75%) is required.
