As aligned with this need, the Medication Management Pain Management Team (MMPMT) leverages the knowledge and skills of Nurse Practitioner (NP) in coordination the Clinical Pharmacist Practitioner (CPP) to collectively provide a high in specialized, coordinated, comprehensive medication management service, to include but not limited to the prescribing or deprescribing of opiods for pain and the assessment and the treatment of OUD. The MMPMNP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients. Providing direct patient care full time and work as a member of a medication management team with a clinical pharmacist to oversee and coordinate medical care for a population of Veterans in a primary care practice setting utilizing the Biopsychisocial Model of Pain Care and VA Stepped Care Model of Pain Care

Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. - Perform initial histories and physical examination on outpatients using Tele-Health and/or in person visits.
- Provide ongoing pain care for assigned patients.
- Prescribe medications within defined scope of practice.
- Utilizes evidence-based approach to medication management for chronic and acute pain.
- Provides evidence-based approach to medication management for Veterans with Opioid Use Disorder (OUD).
- Order and interpretation of diagnostic studies such as laboratory tests, x-ray, magnetic resonance imaging (MRIs).
- Review opioid safety metrics (e.g., PDMP, drug screening, STORM reviews).
- Obtains consent for long term opioid therapy.
- Initiates, responds to and expediate requests for consultations, special tests and studies
- Provide education and counseling of patients and families in pain self-management, active treatment modalities, and use of prescribed treatments and medications.
- Participates in multidisciplinary team meetings such as Pain Board, STORM and Kernersville Interdisciplinary team discussions.

- Education: A post-master's certificate or master's or doctoral degree as a Nurse Practitioner. Must be a graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
- The Accreditation Commission for Education in Nursing (ACEN) or
- The Commission on Collegiate Nursing Education (CCNE).
NOTE: Foreign Education. Graduates of foreign schools of professional nursing programs, the nurse practitioner degree equivalency must be validated through an Agency recognized degree equivalency evaluation agency, such as the International Consultants of Delaware (ICD). Candidates with equivalent nurse practitioner masters or doctorate level degrees who possess current, full, active, and unrestricted registration and certification will meet the requirement of graduation from an approved school of professional nursing to warrant appointment.- License and Certification:

- Basic Certification.
