This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

The Patient Aligned Care Team (PACT) Senior Social Worker participates in the Primary Care Clinic at the VA Medical Center and at Community Based Outpatient Clinics (CBOC). The PACT is a comprehensive team which delivers primary care to Veteran patients in a longitudinal rather than episodic fashion and which has as its focus: prevention; health promotion, coordination, and chronic disease management.

The Social Worker in this Veteran-centric approach primarily functions as a direct-care provider for all psychosocial and behavioral health issues with an assigned panel of patients and participates as the medical Social Worker within the PACT. The PACT Senior Social Worker also functions as a care coordinator to ensure that access to and services provided for healthcare meets the needs of the patient, as defined by the Veteran/Caregiver.

The PACT Senior Social Worker will:

  • Conduct psychosocial and behavioral health assessments in collaboration with the interdisciplinary treatment team, the Veteran patient, family members and significant other.
  • Demonstrate an understanding of the range of treatment and possesses skills needed for all adult patients including geriatric patients and can address issues related to their physical limitations, psychological needs and age-associated illnesses and conditions.
  • Develop the psychosocial treatment plan and set achievable treatment goals with the Veteran/caregiver and/or family, independently establishing this plan and following up as needed.
  • Educate the Veteran and/or caregiver of the available services and assisting them in establishing the appropriate referrals based on the Veteran's preference or that of their authorized surrogate decision-maker.
  • Serve as a liaison with community agencies, assist Veterans (or caregivers) with referrals for services from community and government agencies, and help coordinate these services, when appropriate.
  • Be experienced in making rapid assessments and developing crisis management plans for patients in crisis.
  • Function as a vital member of the interdisciplinary follow-up team for any patients flagged as high risk for suicide and working closely with the PCMHI staff or other Mental Health staff at their clinic for appropriate warm handoffs when needed.
  • Assess for and assist in coordinating services for personal care, long-term care, respite, and other needed services across the continuum of both institutional and non-institutional care.
  • Complete comprehensive psychosocial assessments for patients to help determine when mental health services are needed and make appropriate referrals as outlined by medical center policy.
  • Work with the PACT Social Work Supervisor(s) and Primary Care Social Work Supervisor in planning for expected gains/losses of PACT Social Work personnel as well as assisting PACT Social Work Supervisor(s) in providing and managing coverage needs in their assigned clinic.
  • Function as a trainer for new PACT Social Workers as well as provide ongoing mentoring for the new PACT Social Workers within the first 6-12 months of hire, in conjunction with the PACT Social Work Supervisors.

Pay: Competitive salary and regular salary increases.

Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)

Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.

Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA

Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)

Telework: May be available on an ad-hoc basis as determined by the agency policy.

Basic Requirements:



United States Citizenship:
Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C.

  • 7403(f).

Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to

Licensure:
Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.

Drivers License: A valid driver's license is required as this position requires local travel to provide community-based care to Veterans through the use of a government vehicle or other individual arrangements.

Grade Determinations:

GS-12 Experience/Education: The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a

defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty.

Licensure/Certification: Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able

to provide supervision for licensure.

Demonstrated Knowledge, Skills, and Abilities (KSA's): In addition to the experience above, candidates must demonstrate all of the following KSAs:

  • Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management.
  • Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice.
  • Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes.
  • Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills.
  • Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.



Preferred Experience:
1-year post-MSW experience in an outpatient medical setting

Physical Requirements:
Must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies. Social Worker must be able to perform primarily light and sedentary duties with occasionally moderate physical demands, exercise patience, and control emotions, with reasonable accommodation, if necessary, without endangering the health and safety of the social worker or others.

This position requires a valid state driver's license and local travel to provide community-based care to Veterans. The position requires significant travel in the community, through use of a government vehicle or other individual arrangements, to meet with representatives/stakeholders who aid and support to Veterans in clinics, medical centers, Community Clinics, etc.

This position also requires close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; and repetitive motions of the wrists, hands, and/or fingers.
