Outreach Service Coordinator
The Outreach Coordinator for U.S.VETS is responsible for coordinating the screening and intake process for new clients admitted into housing and programs. This person will also contact and collaborate with local agencies that provide services to the homeless and the housed as well as conduct outreach efforts to those veterans who are currently living on and around the site.
FLSA Classification: Non-exempt
Responsibilities
• Possesses an in-depth, working knowledge of the veteran community
• Establishes and maintains working relationships with local service providers
• Connects veterans experiencing homelessness and at-risk veterans and their families with housing and supportive services
• Maintains professionalism when interfacing with clients and partners in person at and around the site
• Supports and upholds program principles by communicating with the Director, Case Managers, and other staff regarding client issues as needed
• Ensures proper and timely communication with HUD/VASH Case Management and the Veteran Support Coordinator while working with client cases
• Conducts screenings for housing and programs potentially available to clients
• Performs non-clinical assessments, interventions, and crisis resolution with clients
• Facilitates the housing intake and discharge processes at the site
• Identifies goals of potential clients to ensure suitability for Permanent Supportive Housing or other continuum programs
• Tracks and maintains a database of clients served by the site’s outreach process
• Ensures grant and program compliance by verifying eligibility through reviewing documentation such as homeless status, veteran status, income status, etc.
• Works with HUD/VASH Case Managers to compile mandatory housing intake verification documentation and ensures files are complete
• Coordinates consistent and regular outreach efforts with local homeless service providers, agencies, and community service partners
• Plans, executes, and participates in community events
• Maintains an events calendar to share with clients
• Provides regular updates to executive management at the site regarding admissions, discharges, client developments, campus needs, etc. by participating and contributing to weekly staff meetings
• Performs other duties as assigned
Qualifications
Requirements
• Bachelor’s degree in social services field
o Minimum of four years of relevant employment experience may substitute for degree requirements
• Minimum of 2 years of supervisory experience in related field
o Two years of employment at U.S.VETS may substitute for supervisory experience
• Ability to work effectively with a diverse group of clients, staff, and community members
• Excellent written and oral communication skills
• Leadership and conflict management skills
• Demonstration of personal and financial integrity in the workplace
• Ability to take direction, work independently with minimal oversight, and to work within a team
• Valid driver’s license required, must meet company insurance requirements and complete a provided driver training course
