What are the responsibilities and job description for the Nurse Practitioner position at UB Neurosurgery, Inc?
Diagnose illness and physical conditions and may perform corrective measures within her/his specialty area of practice in collaboration and under direction of the surgeon within the practice and practice protocol. Additionally will assist with chart review and quality measures to assure the quality and efficacy of the practice.
Primary Duties and Responsibilities:
1. Assess health status of individuals in OP environment by eliciting health and medical histories, perform physical exams ordering appropriate diagnostic tests and identifying abnormalities from written reports and studies which require action by coordinated physician. Perform pre-op and post op calls to patients and Attendings. Perform procedures within scope of practice and upon approval of attending i.e. ESI.
2. In the inpatient environment, monitor health status of patients by performing daily clinical rounds and coordinate care with attending, fellows and residents. Meet with families and patients to discuss health status and plan appropriate care provision and discharge coordination Management of medical problems of patient’s on service, reviews co-morbid conditions to assure proper management. Implements correct plan of care-update attending of record. Daily review of charts of new admissions to insure correct adherence to quality documentation and standards of care, meeting with discharge planning to decrease length of stay and optimize care of our patients.
3. Issue prescriptions for drugs, devices and therapies in accordance with scope of practice and physician protocols.
4. Perform neurophysiological testing as required by protocol and document appropriately.
5. Assist with patient phone calls, suture removal, consents as needed.
6. Obtain informed consent as needed for surgeries, studies and procedures.
7. Provide education, instruction and counseling to patients, families and groups of individuals. Including physician, professional and community groups.
8. Maintain appropriate billing information and monitor adequate completion of patient note.
9. Coordinate care with patient primary physician and other agencies as necessary.
10. Document appropriately and accurately in a timely fashion the patient interaction, care and treatment of patient.
11. Assist in quality monitoring reviews for the practice. Including coordination of M&M with residents and fellows. Assist with NYPORT reviews and QA reporting to hospital as identified.
12. Perform chart reviews to assure quality of documentation.
13. Recommend practice procedural changes in coordination with clinical coordinator (Office Manager) and administration.
14. IP consult and other Kaleida facility consult in coordination with attendings.
15. Arrange and coordinate patient transfers form outside facilities.
16. Follow-up with PMD’s rehab MD’s weekly for coordination of care.
17. Other duties as requested.
Qualifications
- Valid Nurse Practitioner license
- Proficiency in patient assessment and care
- Experience with EHR systems and discharge planning
- Skills in catheterization, IV insertion, and tube feeding
- Knowledge of physiology and basic life support
- Familiarity with premium skills such as Epic and critical care experience
- Relevant experience in gastroenterology and assisted living settings
