Patient Outreach Coordinator : UBMD Primary Care seeking a part-time Patient Outreach Coordinator who will be responsible for patient outreach via phone with a focus on scheduling annual visits and gaps in care utilizing clinic rosters. The Patient Outreach Coordinator will be remote after a training period and candidates will need to be onsite for weekly / monthly meetings. Qualified candidates should have a minimum of 1-2 years of customer service experience, preferably in health care. Candidates should also have experience with excellent communication, organizational, customer service, and multi-tasking skills.
Pay range is $17-$18 / hour. Any offer of employment is contingent upon a successful background check and drug screen. AA / EOE

Job Summary :
The Patient Outreach Coordinator (POC) will be responsible for patient outreach via phone with a focus on scheduling annual visits and gaps in care utilizing clinic rosters.
Essential Functions :
- Ensures accurate and timely scheduling of annual health maintenance visits for patients.
- Understands insurance guidelines in order to properly schedule annual health maintenance visits (insurances via calendar year versus insurances that require 366 days).
- Understands continuity and a large range of physician preferences from various locations.
- Schedules annual appointments utilizing specific codes (PANN, PAWV, PVAT, WELL, WEL2).
- Provides patients with reminder calls.
- Routes calls and tasks to office when appropriate.
- Organizes a large volume of tasks and proper reactivation to allow follow up in 30-day increments.
- Accurately manages and color codes rosters.
- Works with the Director of Clinical Operations and Operations Manager on office guidelines regarding timing of correspondence to patients including annual importance letter, a Step2 letter and then possible discharge letter based on physician guidance, when necessary.
- Coordinates patient reminders via phone for gaps in care, if applicable, based on a specific roster and / or alerts in chart from practice facilitation team (i.e. reminder to schedule mammogram, colonoscopy, diabetic eye exam, etc.).
- Reviews and addresses daily tasks as assigned.
- Complies with all OSHA regulations when present at any UBMDFM clinic.
- Adheres to HIPAA and confidentiality policies and procedures.

Education : High School graduate or equivalent required.
Experience setting : Minimum of 1-2 years’ customer service experience required, preferably in a health care
Knowledge, Skills & Abilities : Excellent customer service, communication and multi-tasking skills required.
