Health Unit Coordinator (HUC), Night Shift, Skilled Nursing Facility (SNF) 2 North

UC Health is hiring a full-time HUC, for SNF 2 North, for the night shift at Daniel Drake Center for Post-Acute Care.

The HUC will support Daniel Drake's SNF 2 North department, delivering high-quality care to patients based on assessed needs, established standards of care and according to policy and procedures.

Responsibilities

The Health Unit Coordinator (HUC) facilitates the efficient functioning of a patient care unit by performing receptionist and clerical/non-clinical support activities. The Health Unit Coordinator has knowledge of age-specific needs of the patient population.

• Support-provides clerical/non-clinical support for the provision with patient services under the supervision of the nursing management. Supports the patient care environment by communicating clearly, courteously and effectively, verbal and through written and computer documentation, while maintaining confidentiality of information.
• Assembles the patient chart with appropriate forms according to policy, providing additional forms as necessary. Prepares charts for patients scheduled for the operating room, special studies/therapies, transfer or discharge. Prepares maintains and files forms and reports in patient charts. Dispositions patients through appropriate computer systems to promptly process admissions, discharges, deaths and transfers.
• Facilitates scheduled studies and therapies by scheduling and coordinating in-house transportation for patients. Assists in coordination of discharge process. Should process discharges appropriately, assuring that all of the patient's records is sent to the Medical Records department.
• Keeps the Admitting office informed of patient activity on the unit through the Hospital information system. Communicates with the Environmental and Nutritional Services to ensure appropriate activities occur.
• Facilitates interdepartmental communication by providing information (e.g. phone messages) to appropriate clinicians.

Qualifications

Education and Experience Requirements:

• High School Diploma or GED
• 1-2 years equivalent customer service experience
• HUC education from college or vocational school preferred
