Primary Purpose:
Participates in the collection of data utilized when developing the M.D. S. for each resident. Responsible for the completion and transmission of the M.D.S. Responsible for the development of the care plan after completing the triggers.

Qualifications
Knowledge and Skills:
Education: Graduate of an approved RN program.
Required Skills and Knowledge:The ability to communicate effectively both verbally and in writing, especially with older adults is required
