Job Title: Licensed Practical Nurse (LPN) - Part-time, Traveling
The Licensed Practical Nurse at UCAN addresses medical care coordination needs of youth in the Partners in Parenting (PIP) program and community settings.
Essential Functions:
- Trains staff and youth on various healthcare topics.
- Maintains accurate case notes for youth.
- Assists with completion of Asthma Action Plans.
- Develops service plan goals for youth with high-risk/chronic health needs.
- Educates youth on prenatal care, fetal development, and family planning options.
- Reviews prescriptions and medication dosages for youth on psychotropic medications.
- Covers psychiatric and psychological appointments.
- Assists with medication refills for chronic conditions.
- Administers hearing and vision screenings.
- Plans and implements health management protocols for youth with chronic health issues.
- Liaisons with community hospitals to coordinate youth care.
