Position Title: Licensed Practical Nurse (LPN) - (Part-time, Traveling)

Job Summary: The Licensed Practical Nurse addresses medical care coordination needs of youth within PIP program and community settings.
Key Responsibilities:
- Trains staff and youth on various health care topics.
- Keeps accurate and complete case notes on youth.
- Assists team with completion of Asthma Action Plans.
- Assists with development of service plan goals for youth and children with high-risk/chronic health care needs.
- Educates youth on importance of attending routine prenatal care and effects of cigarettes/drugs/alcohol on unborn fetus.
- Educates youth on all family planning options highlighting pros and cons of each.
- Reviews all prescriptions and medication dosages for youth on psychotropic meds. Provides education to team regarding potential side effects and coordinates with placement provider if needed.
- Coordinates psychiatric and psychological appointments.
- Assists youth with medication refills (e.g. high blood pressure, eczema, diabetes, etc.).
- Administers hearing and vision screenings for youth and their children.
- Plans and implements health management protocols for youth with chronic health problems.
- Liaisons with community hospitals to coordinate care for youth and young adults.
- Promotes and assists in control of communicable diseases through preventive immunization programs, early detection, surveillance and reporting of contagious diseases.

- Performs other related tasks as required or assigned.
Qualifications:
- State of Illinois LPN licensure.
- Current CPR certification.
- Two (2) years' experience in related work.
Special Knowledge and Qualifications:
- Proven ability to work effectively with young people.
- Understands trauma-informed practice.
- Effective verbal and written communication skills.
- Ability to work both independently and part of a high functioning team.
- Experience and ability to use computers for data entry, communication and report writing.
- Car and valid Illinois driver's license and insurance required.
