Position Title: Yv LifeSet Case Specialist
Department: Yv LifeSet Department
Reports to: TLP Supervisor
Position Objective and Summary:
The YV LifeSet Case Support Specialist will assist YV LifeSet youth by providing transportation to assigned destinations while supporting positive youth interactions during community appointments and community events. The YV LifeSet Case Support Specialist will provide community mapping education and resources to help educate youth on how to identify and locate community resources prior to their transition out of care. The YV LifeSet Case Support Specialist will offer mentoring and life skill education and support in various areas of positive youth development. The YV LifeSet Case Support Specialist will also assist with file maintenance and basic administrative duties to support the daily operations of program.
Essential Functions and Responsibilities:
- Provide transportation to appointments and community events for youth assigned
- Monitor high acuity youth’s interactions while in the community to ensure appropriate and safe boundaries and limits
- Provide community base mapping education while transporting youth around to show them their local neighborhood community base resources such as the grocery store, hospital, police station, bus routes etc.
- Participate and assist with leading youth’s recreational activities and outings.
- Transport and role model appropriate community and social interaction while escorting high acuity older youth for psychiatric, psychological evaluations, and doctors’ appointments when necessary.
- Transport and assist pregnant and parenting youth to prenatal and childcare appointment when necessary.
- Perform office duties such as printing, copying, faxing, scanning and direct mail
- Complete filing for the legal case management team
- Assist with prepping for YV LifeSet, Utilization reviews, and COA
- Keep accurate and complete documentation on client interactions and service delivery.
- Become familiar with team members’ responsibilities and the opportunities and challenges. presented by the youth served in the program.
- Participate as a member of the YV LifeSet team to provide administrative support to YV LifeSet team members as assigned by Manager.

Education/Job Experience/Certification:
- Bachelors Degree Preferred.
- Car and valid Illinois Drivers license and Insurance required.
Special Knowledge and Qualifications:
- Ability to maintain confidentiality and classified information.
- Ability to communicate with older adolescents, young adults, and children in a courteous and professional manner.
- Ability to work with high acute older adolescent with mental health and severe behaviors.
- Ability to work independently and as part of a high functioning team.
- Experience and the ability to use computers for data entry, communication and report writing.
- Ability to transport children in accordance with IDCFS regulations.
- Ability to drive and extended distance if necessary.
Work Environment: This position required local travel to provide services to youth at the TLP placements and in their community.
