Description
UCFS is looking for candidates who are passionate about making a difference in the lives of others! We are currently seeking a per-diem Registered Nurse who will utilize critical thinking skills to interpret complex information in a multidisciplinary setting to guide patients and families to health and wellbeing across the continuum of care in an outpatient setting.
Responsibilities-
- Provide broad range of nursing support for large outpatient practice to include nursing consultation, assessment, patient follow up and patient surveillance of health statuses and outcomes.
- Assists with physical exams, completes required screenings and laboratory testing and administers treatments / immunizations / medications to clients
- Provides client services; educate client / parent as needed; assists the client with prescription needs / refills coordinates services with / for the client; acts as a client advocate to pursue financial assistance, initiates referrals to community-based organizations as needed (Home Care, Healthy Start, Dental, and Behavioral Health)
- Responds to calls from patients, pharmacies, home care agencies and family members; collaborates with school nurses, DCF and other providers as needed. Completes paperwork needed by external organizations
- Works collaboratively with behavioral health and dental department to facilitate integrated care for patients
