provide family engagement and support services to families and children with or at risk of developmental, social, and emotional disorders; help individuals, families, and workers from state and private organizations. You will gain access to UCFS for comprehensive evaluations, consultation and treatment.
Responsibilities
- Serve as a liaison between referral sources (internal and external) and individuals/ families seeking assistance and BHS/ through telephone and face to face outreach and engagement
- Completes or directs the completion of in house and external referrals as needed and tracks client/family follow up as needed. Provides care coordination with pharmacies, providers, hospitals, higher levels of care and family members and other departments as needed.
- Monitor and schedules client appointments as needed; verify Husky insurance and other payer information as needed.
- Completes intake and creates OCHIN EPIC accounts for new clients and updates existing OCHIN EPIC with current information when needed.
- Collect releases of information and outreach to client’s previous or current providers and collect all relevant medical and behavioral health treatment information, evaluations, educational records and review prior to evaluation; completing summary of findings for providers.
Requirements
- Bachelor’s Degree in Human/Social Services or related field
- 1 to 3 years of experience in providing behavioral health case management
- Strong communication and interpersonal skills
