UChicago Medicine AdventHealth is hiring for their Community House Calls program. This is a growing program which is so needed in our communities. This program serves our homebound patients, giving them access to primary care in the comfort of their homes.
- Experienced APP's preferred, but new grads welcome to apply
- Weekly Monday - Friday schedule
- No call requirement
- Patient visits in assigned territory
- Outpatient care in patients homes
- Resources available for labwork, imaging, procedures, etc.
- Working collaboratively with Physicians, Social Workers and Palliative Care
- Benefits, CME and occurrence-based malpractice insurance
- Annual Salary : $105k - $118k
