The Social Worker would be working closely with the established health psychology integrated care program to expand the clinic's behavioral health integration resources to include a Collaborative Care Model program. This position will focus on mental health needs for patients in Primary Care and is a new way to envision social work within a clinic setting.
In this role, the clinical Social Worker would be a part of the robust social work and embedded health psychology communities in the Department of Psychiatry and Behavioral Neuroscience. The health psychology program in the Practice is a training site for health psychology interns, externs, and post-doctoral fellows. The primary care providers are experienced working with behavioral health specialists and the clinic has an established program focused on behavioral health integration.

UChicago Medicine is committed to transparency in compensation and benefits. The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.
The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.

Essential Job Functions
- Psychosocial and Safety Assessments
- Transitions of Care Planning
- Coordination of interdisciplinary interventions
- Crisis intervention
- Supportive counseling around adjustment to illness, hospitalization, and/or outpatient care
- Promote access to hospital and community resources
- Master's degree in Social Work from a School of Social Work accredited by the Council on Social Work Education
- Licensed Social Worker (LSW)
- Self-motivated, organized and comfortable in a fast paced, dynamic environment
- A Collaborative, solution-focused, team player to help facilitate safe patient outcomes and contribute to institutional goals
- Previous clinical social work experience in a health care setting of at least one year
- Licensure in Illinois
- Licensed Clinical Social Worker (LCSW)
- Previous clinical social work experience in an ambulatory care setting
- Job Type/FTE: Full Time - 1.0 FTE
- Shift: Days - 8-Hr Shift- Full time- fully remote- needs to be able to come onsite monthly for meetings/requests.
